There are two general skin release forms that are in common use. One was designed by our state organization several years ago, the other was designed by the National Federation of State High School State Associations, and is required for high school and middle school competition.
The correct skin release form to use for Kids Division events is the first one I described. The easiest way to determine which one you have is to look at the silhouette figures. If they're off to one side of the page, it's the high school form. If the two figures are on both sides of the page, that is the Kids form (the one you should use).
There is no fixed time period required by the rules, although the high school form has recommendations.
